Aftercare & Transitions from Treatment
Support for the space between treatment and everyday life.
Leaving residential treatment, a partial hospitalization program (PHP), or an intensive outpatient program (IOP) can bring relief, hope, and a lot of uncertainty. In treatment, support is built into the day. At home, school, work, relationships, and familiar stressors come back into the picture and everyone is figuring out what happens next.
At Whole Human Wellness Therapy, we help teens, young adults, adults, and families navigate that transition with connection, structure, and practical support. Together, we work on bringing the skills and insights from treatment into the moments when they are needed most.
You don’t have to have the next chapter figured out before reaching out.

Going from several hours of treatment each day to one therapy session a week can feel like a significant shift. Some clients are ready for that rhythm. Others benefit from more frequent contact and additional support as they adjust.
Depending on your needs and clinician availability, aftercare at WHW may include:
Multiple therapy sessions per week to provide additional connection and opportunities to practice skills during the transition.
Individual therapy to continue working on the concerns, relationships, and patterns that brought you to treatment.
Family therapy and parent support to help everyone understand their role and respond to challenges with greater consistency.
Executive functioning support to help with planning, organization, routines, and follow-through, including in-home services when appropriate.
Care coordination with your treatment team, psychiatrist, school, and other providers, with appropriate consent.
Aftercare planning that translates broad recommendations into specific, realistic next steps.
We work with you to determine a starting point and revisit the plan as your needs change.

Knowing a skill and using it in the middle of a difficult moment are different things.
You might understand how to set a boundary but struggle to hold it when your teen becomes upset. You may leave treatment motivated to return to school, then feel overwhelmed on the first morning. Or you may recognize an old pattern and still need help responding differently.
These are the everyday moments we work on together.
Our work may include:
Building manageable routines around school, work, appointments, and daily responsibilities.
Practicing coping and communication skills in situations that come up between sessions.
Navigating school reentry and coordinating with school supports.
Clarifying expectations, responsibilities, and boundaries at home.
Supporting independence while identifying where help is still needed.
Recognizing early signs that things are becoming harder and deciding how to respond.
Making room for friendships, interests, connection, and life beyond treatment.
The goal is to make progress something you can keep practicing in your actual life.
-
When a loved one returns home from treatment, families often feel both hopeful and on edge.
How much should you check in? What should you do when they withdraw? Which expectations should stay firm? How do you support independence while taking concerns seriously?
We help families talk through these questions and create a shared understanding of what support will look like at home.
Together, we may work on:
Clear, realistic household expectations.
Communication during conflict or distress.
Boundaries parents and caregivers can follow through on.
A balance of connection, accountability, and growing independence.
Plans for responding to setbacks and seeking additional support.
Ways to rebuild trust through consistent actions over time.
For adult clients, family involvement is guided by the client’s consent, preferences, and treatment needs.
-
You do not have to wait until treatment ends to start building your outpatient support.
When appropriate, we can connect with your treatment team before discharge to understand recommendations, identify priorities, and begin planning for the transition home.
We also offer adjunct family therapy while a client is participating in residential treatment, PHP, or IOP, when permitted by the program and coordinated with the existing team. This can give families space to work on communication, expectations, and home routines while their loved one receives treatment.
With appropriate releases in place, collaboration helps everyone work toward a more connected plan.
-
Sometimes discharge brings more questions than answers. Family members may disagree about readiness. School may feel overwhelming. Motivation may fluctuate. The plan that sounded manageable in treatment may need adjustments once everyone is home.
Bring us the complicated stuff.
We get curious about what is getting in the way: the person’s needs, their environment, their relationships, and the patterns that emerge under stress. From there, we help turn “we don’t know what to do” into specific next steps.
If concerns suggest that more intensive support may be needed, we help you think through those next steps and coordinate with appropriate providers. WHW provides outpatient services; the level and frequency of care are based on clinical fit.
-
We welcome collaboration with residential programs, PHPs, IOPs, psychiatrists, schools, and other providers seeking outpatient support for a client or family.
Our services may include individual therapy, family therapy, parent support, executive functioning services, and more frequent outpatient sessions when clinically appropriate and available.
We value clear communication about discharge recommendations, ongoing needs, family dynamics, and the practical challenges a client may face after treatment. With client authorization, we work alongside your team to support continuity of care.
-
Whether you are planning ahead for discharge, recently home, or realizing your current support needs an adjustment, we can help you explore what comes next.
You can reach out as a client, parent, caregiver, or referring provider. We’ll talk through your needs, answer questions, and help determine whether WHW is a good fit.
